Independent occupational therapy support following hospital discharge. Adapt OT accepts referrals from consultants, nurses, case managers, therapists, home healthcare teams and other healthcare professionals across the UAE.

- Adults returning home after illness, injury, surgery or hospital admission
- People experiencing falls, unsafe transfers or difficulty with daily activities
- Clients who may need mobility, bathroom, seating, transfer or manual-handling equipment
- Wheelchair users or people facing access barriers at home
- Families and carers who need independent clinical guidance
Diagnoses and referral situations
- Neurological conditions, including stroke, acquired brain injury, Parkinson’s disease and progressive neurological disorders
- Orthopaedic recovery after fractures, joint replacement or surgery
- Frailty, recurrent falls, deconditioning or risk of avoidable readmission
- Complex disability, wheelchair access, seating or pressure-care concerns
- Carer strain, unsafe transfers or manual-handling risk
Assessment pathway
In-person home assessment: our core service for physical assessment, measurements, equipment trials and a detailed review of transfers and the home environment.
Optional virtual consultation: available as an initial step when appropriate.
What happens after referral
- We review the referral and contact the patient or family after consent has been confirmed.
- We aim to respond to new referral enquiries within one working day.
- We arrange the earliest clinically appropriate home assessment, subject to location, urgency and availability.
- We complete the assessment, explain immediate priorities and, where commissioned, provide a clinically justified written report.
What the referrer receives
- Independent recommendations with no equipment-sales conflict
- Clear priorities for safety, function, transfers, falls prevention and carer support
- Equipment-first guidance, with adaptations recommended when clinically required
- Feedback and documentation shared with appropriate consent
- We normally provide written reports within 48 hours of the completed assessment.
